This website is operated by Prime Adult Family Home (Prime AFH LLC), 1215 E Casino Rd, Everett, WA 98203. This policy explains how the website handles information. It does not cover resident records or care services, which are handled directly by the home.
Information You Choose to Share
The inquiry form on the contact page asks only for:
- your name, phone number, and email address;
- how you prefer to be contacted, and what you are interested in (for example, a tour);
- an optional message.
Please do not send medical records, diagnoses, Social Security numbers, insurance details, or other sensitive health information through the website. Care needs can be discussed directly with the home by phone.
How the Inquiry Form Is Delivered
When you press “Send Inquiry,” your entries are sent over an encrypted (HTTPS) connection from your own browser to Web3Forms, a form delivery service, which emails them to the home. They do not pass through this website’s own server, and this website does not keep a copy in a database. Web3Forms handles the message only to deliver it. Inquiries are then kept in the home’s email account (Primeafh@gmail.com, provided by Google).
Information you send is used to respond to your inquiry. This website does not sell information or share it with advertisers.
Phone and Email Links
Phone and email links open your device’s phone or email app. Calls and emails go directly to the home and do not pass through this website.
Cookies, Analytics, and Tracking
This website does not use analytics, advertising trackers, or cookies of its own. Fonts are served from this website rather than from a third-party font service.
Hosting
The website is hosted by Vercel. Like most web hosts, Vercel processes technical information such as your IP address and browser type in order to deliver pages and protect the service.
Maps and Directions
The map on the contact page is provided by Google Maps and loads as you scroll to it. When it loads, Google may collect information such as your IP address and set its own cookies under Google’s privacy policy. “Get Directions” links open Google Maps in a new tab.
Links to Other Websites
Some links, such as the home’s record on the Washington DSHS website, lead to sites operated by others. Their own privacy policies apply there.
